Each Membership Type can have it's own Sign Up form, Renewal form and Update Details form.
Provide your sign up fields
Please provide a list of all the fields needed on the signup forms for each Membership Type, with details as follows:
Field: The label of the field
Description: A description of the field
Update: Whether the field is shown on the update details form as well
Renewal: Whether the field is shown on the renewal form as well
